2014 Arts & Crafts Festival Craft/Exhibitor Application

Exhibitor Fees for 2014, NO REFUNDS once application is processed:
Completed application sent by December 31, 2013: $75.00 per 10'w x 10'd space
Completed application sent from January 1 to March 31, 2014: $100.00 per 10'w x 10'd space
ALL applications sent after April 1, 2014: $150.00 per 10'w x 10'd space

NO application will be processed unless it includes the following:

1) Completed application form
2) Certificate of insurance for 1 million dollars of property damage and bodily injury. 
3) Exhibitor’s FEE (see above)
4) A photograph representative of your display items
5) Self-addressed stamped envelope IF you CANNOT receive space confirmation via e-mail.
